This is a fully remote position, open to applicants in Texas.
• Assist in the development, evaluation, and coordination of comprehensive care management activities.
• Create or aid in the creation of customized service care plans for long-term care members.
• Inform members and their families/caregivers about available services and benefits.
• Assess member needs and available resources; propose or facilitate plans aimed at achieving optimal outcomes.
• Formulate ongoing long-term care plans and identify appropriate providers, specialists, and community resources.
• Act as a liaison between members, families, caregivers, and care provider teams to ensure service accessibility.
• Offer resource support for employment, housing, participant direction, independent living, justice, and foster care requirements.
• Oversee care plans, member status, and outcomes; suggest updates based on evolving member needs.
• Engage with long-term care healthcare providers and partners.
• Gather, document, and maintain member information and care management activities to ensure regulatory compliance.
• May conduct on-site visits to evaluate member needs and collaborate with providers or resources.
• Facilitate education regarding procedures, healthcare provider instructions, service options, referrals, and healthcare benefits.
• Provide leadership feedback on opportunities to enhance care quality and service delivery.
• Adhere to all policies and standards; perform additional duties as required.
• Bachelor's degree with 1 year of relevant experience OR equivalent experience gained through applicable knowledge, duties, scope, and skills reflective of this position's level.
• Proficiency in Microsoft Office Suite, including Excel, OneNote, PowerPoint, and Outlook.
• Strong skills in problem-solving, organization, time management, and verbal communication.
• Ability to quickly adapt to changing priorities while maintaining flexibility, accuracy, and professionalism.
• Comfort in a 100% inbound phone-based environment with a high volume of daily calls.
• For Superior Health Plan Only: Direct experience working with individuals with disabilities and/or vulnerable populations with chronic or complex conditions, including children and young adults within three of the last five years.
• Availability to work Monday–Friday, 9:30 a.m.–6:00 p.m. CST.
• Occasional weekend training may be necessary.
