This is a fully remote position, open to applicants in Texas.

📋 Description

• Design, evaluate, and manage comprehensive care management initiatives.

• Create or assist in formulating tailored service care plans for long-term care clients.

• Inform members and their families/caregivers about accessible services and benefits.

• Assess member requirements and available resources, recommending or facilitating plans for optimal outcomes.

• Establish ongoing long-term care strategies and identify relevant providers, specialists, and community resources.

• Facilitate coordination among members, families, caregivers, and care provider teams.

• Offer resource support pertaining to employment, housing, participant direction, independent living, justice, and foster care needs.

• Oversee care plans, member status, and results, suggesting updates as necessary.

• Engage with long-term care providers and associated partners.

• Gather, document, and uphold member data and care management activities for regulatory adherence.

• May conduct on-site visits to assess the needs of members.

• Educate on procedures, provider instructions, service options, referrals, and healthcare benefits.

• Provide leadership insights on enhancements in quality of care and service delivery.

• Carry out additional tasks as assigned.

• Adhere to all policies and standards.


⛳️ Requirements

• Must be a resident of Texas.

• Bachelor’s degree with a minimum of 1 year of relevant experience.

• Equivalent experience may be accepted based on applicable knowledge, responsibilities, scope, and skills reflective of the position level.

• Direct experience working with individuals with disabilities and/or vulnerable populations facing chronic or complex conditions, including children and young adults, within three of the last five years.

• Additional state-specific requirements may be applicable.

• Familiarity with Medicare/Medicaid programs is advantageous.
