This is a fully remote position, open to applicants in Iowa.
• Responsible for developing, evaluating, and coordinating comprehensive care management activities for members in long-term care.
• Assists in creating personalized service care plans and educates both members and caregivers about the range of available services and benefits.
• Assesses member needs and available resources, recommending or facilitating suitable care plans.
• Identifies providers, specialists, and community resources relevant to long-term care.
• Coordinates efforts among members, caregivers, and care provider teams to ensure accessibility of services.
• Offers resource support addressing employment, housing, participant direction, independent living, justice, and foster care requirements.
• Monitors care plans, member status, and outcomes, suggesting updates in response to member needs.
• Engages with long-term care providers and partners.
• Collects, documents, and maintains member information and care management activities for compliance with regulatory standards.
• May perform on-site evaluations to assess member needs and collaborate with relevant providers or resources.
• Provides education to members and caregivers concerning procedures, provider instructions, service options, referrals, and healthcare benefits.
• Delivers leadership feedback on opportunities to enhance quality of care and cost-effective service delivery.
• Performs other assigned responsibilities.
• A Bachelor's degree is mandatory, along with a minimum of 1 year of experience with the populations served under the Iowa plan.
• Alternatively, an RN with over 6 years of experience working with the populations served for the Iowa plan is acceptable.
• Requires a Bachelor's degree and 1 year of relevant experience, or equivalent experience gained through applicable knowledge, duties, scope, and skills.
• For populations with Physical Disabilities/Elderly, must meet one of the following criteria: RN licensed in Illinois; a bachelor's or master's degree in a human services-related field; or LPN with one year of experience in conducting comprehensive assessments and delivering formal services for the elderly.
• For populations with Brain Injury/HIV/AIDS, must satisfy one of the following: Illinois-licensed RN with a specified bachelor's degree or four years of case management experience; certified or licensed social worker with specified bachelor's or master's degree; or unlicensed social worker with a bachelor's degree in social work, social sciences, or counseling.
• Experience with Brain Injury/HIV/AIDS populations, particularly concerning addictive and dysfunctional family systems, racial and ethnic minorities, homosexuals and bisexuals, individuals with AIDS, and substance abusers.
• For the Superior Health Plan, direct experience with individuals with disabilities and/or vulnerable populations with chronic or complex conditions, including children and young adults, within three of the last five years is required.
