This is a fully remote position, open to applicants in Connecticut, +2 more states.

📋 Description

• Handle incoming calls from members concerning benefit inquiries, care coordination, program questions, connecting them to their care manager, and conducting screening assessments to identify medical intervention risk factors.

• Document calls accurately and contemporaneously in the specified PHI system.

• Manage a designated caseload of members requiring short-term care coordination and health navigation.

• Collaborate with the interdisciplinary care team to ensure timely communication regarding member issues or needs, as well as monitor member screenings to enhance quality and cost outcomes.

• Conduct outreach calls to members to verify that services are properly in place.

• Undertake additional responsibilities as assigned.

⛳️ Requirements

• Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or equivalent relevant medical training or work experience.

• High School diploma or GED.

• Work experience that demonstrates effective verbal and written communication skills, including a strong grasp of grammar, spelling, and punctuation.

• Experience in a fast-paced setting that necessitates managing multiple priorities at once.

• Associate’s Degree is preferred.

• Previous work experience in a call center, care management, or healthcare setting, or familiarity with medical terminology, Medicaid, and Child Health Plus benefit products is preferred.

• Strong telephone, organizational, and customer service abilities.

• A team player who is organized and detail-oriented.
