LPN, New York License Required – Care Navigator

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct telephone consultations with patients participating in care management and/or remote patient monitoring programs.

• Offer support and education regarding patients' chronic illnesses.

• Inform patients about their health conditions and equip them with strategies for lifestyle and behavior modifications.

• Assist patients in establishing and achieving goals that align with provider-approved care plans.

• Keep accurate and current patient records, documenting interactions and care plans in accordance with protocol.

• Address patient concerns and obstacles to care, finding practical solutions to enhance adherence and outcomes.

• Communicate with patients in a clear, compassionate, and effective manner.

• Adhere to approved workflows for relaying patient needs to healthcare providers.

• Engage in training sessions, team meetings, and initiatives aimed at quality improvement.

• Respond to remotely transmitted patient data, such as blood pressure, blood glucose, weight, and pulse oximetry, following established partner workflows.

• Build relationships with assigned patients to empower them in managing chronic illnesses and improving health outcomes.

⛳️ Requirements

• Must possess a current, valid, and good standing New York Nursing License (LPN/LVN).

• Additional state licenses may be necessary.

• At least 3 years of experience in primary care, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.

• Strong communication and interpersonal abilities.

• Exceptional organizational and time management skills.

• Proficiency in electronic health records (EHR) systems and care management software.

• Capable of working independently as well as collaboratively within a team.

• Empathetic with a patient-centered approach to care.

• Must have a reliable internet connection.

• HIPAA-compliant workspace is required.

• Familiarity with virtual communication platforms like Zoom and Slack.

• Additional Compact Nursing License is preferred.
