This is a fully remote position, open to applicants in New York.
β’ Conduct telephone consultations with patients participating in care management and/or remote patient monitoring programs.
β’ Offer support and education regarding patients' chronic conditions.
β’ Inform patients about their health conditions and empower them with strategies for lifestyle and behavior modifications.
β’ Assist patients in establishing and achieving goals aligned with provider-approved care plans.
β’ Maintain precise and current patient records and document interactions and care plans following established protocols.
β’ Address patient concerns and obstacles to care, providing practical solutions to enhance adherence and outcomes.
β’ Deliver clear, compassionate, and effective communication to patients.
β’ Adhere to approved workflows for relaying patient needs to providers.
β’ Engage in training sessions, team meetings, and quality improvement initiatives.
β’ Respond to remotely transmitted patient data, such as blood pressure, blood glucose, weight, and pulse oximetry, in accordance with approved partner workflows.
β’ Foster relationships with assigned patients to support chronic illness management and enhance health outcomes.
β’ Must possess a current, valid, and in good standing New York Nursing License (LPN/LVN).
β’ Additional state licenses may be necessary.
β’ A minimum of 3 years of experience in primary care, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring is required.
β’ Strong communication and interpersonal skills are essential.
β’ Exceptional organizational and time management skills are necessary.
β’ Proficiency in electronic health records (EHR) and care management software is required.
β’ Ability to work both independently and collaboratively within a team.
β’ Empathy and a patient-centered approach to care are crucial.
β’ A reliable internet connection is mandatory.
β’ A HIPAA-compliant workspace is required.
β’ Familiarity with virtual communication platforms like Zoom and Slack is essential.
β’ Residency in New York is required for the posted pay range; compensation for other states will be aligned with the state of residence.
