This is a fully remote position, open to applicants in United States.
β’ Conduct phone consultations with patients involved in care management and/or remote patient monitoring programs.
β’ Offer support and education regarding patients' chronic conditions.
β’ Instruct patients on health issues and empower them with strategies for lifestyle and behavior changes.
β’ Help patients establish and achieve goals in line with provider-approved care plans.
β’ Ensure patient records are accurate and up-to-date, documenting interactions and care plans in accordance with protocols.
β’ Address patient concerns and barriers to care, identifying practical solutions to enhance adherence and outcomes.
β’ Deliver clear, compassionate, and effective communication to patients.
β’ Adhere to approved workflows for communicating patient needs to providers.
β’ Engage in training sessions, team meetings, and quality improvement initiatives.
β’ Respond to remotely transmitted patient data, such as blood pressure, blood glucose, weight, and pulse oximetry, following approved partner workflows.
β’ Manage an assigned group of patients throughout the month and assist with additional patients or tasks as required.
β’ A current, valid, and in good standing Multistate/Compact Nursing License (LPN/LVN) is required.
β’ Additional state licenses may be necessary.
β’ Over 3 years of experience in primary care practice, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring.
β’ Strong communication and interpersonal skills are essential.
β’ Excellent organizational and time management capabilities.
β’ Proficient in utilizing electronic health records (EHR) and care management software.
β’ Ability to work independently as well as collaboratively within a team.
β’ Empathetic and patient-centered approach to care is needed.
β’ A reliable internet connection is required.
β’ HIPAA-compliant work area is necessary.
β’ Proficiency with virtual communication tools (e.g., Zoom, Slack) is expected.
β’ Availability to work Monday through Friday, 9:00 am to 5:30 p.m. Eastern Time, unless otherwise specified.
β’ Additional state licenses may be required and will be reimbursed by HealthSnap.
