Licensed Practical/Vocational Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in accordance with the Nurse Practice Act of the state.

• Execute and adhere to personalized care plans developed by the RN.

• Educate patients and their families on self-care methods and information related to diagnoses.

• Accurately document care provided in a timely manner.

• Collaborate with the care team—encompassing OT, PT, HHA, and other disciplines—and relay patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely while following aseptic techniques.

• Carry out additional duties as necessary for patient services.

⛳️ Requirements

• Graduate from an accredited practical or vocational nursing program.

• Current LPN or LVN license in California.

• Over 3 years of nursing experience; preference for home health or med/surg background.

• Valid CPR certification from AHA.

• Possess a valid driver’s license, a reliable insured vehicle, and the ability to travel to patient residences.

• Excellent communication skills and a collaborative, patient-centered approach.
