Licensed Practical Nurse – Primary Care

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Support onsite MA, LPN, and RN by collaborating with the provider and healthcare team via the EMR.

• Manage the patient task list through the EMR system.

• Relay clinical results and educational information to patients through phone or portal, ensuring documentation in the EMR.

• Execute medication prior authorizations.

• Record interactions with patients and families, along with medications and test results in medical records.

• Aid the care team with nursing responsibilities through the EMR.

• Maintain close communication with nursing and provider teams.

• Guarantee that patient information is accurately reflected in charts for quality reporting and data extraction.

• Oversee and coordinate provider-seen patient care, including the tracking of tests and referrals.

• Keep track of insurance requirements and guidelines.

• Serve as a patient advocate.

• Monitor patients to encourage wellness, health maintenance, preventive care, and management of chronic conditions.

• Distribute patient education materials and communicate provider advice and instructions effectively.

• Organize, schedule, and manage specialist referrals and services ordered by providers.

• Strong ability to multitask.

• Excellent organizational skills.

• Proficient communication skills.

• Completion of an accredited Vocational/Technical Nursing Program.

• Active Kansas nursing license without restrictions.

• BLS certification required.

• Preferred: 1–2 years of experience in a clinic environment.
