This is a fully remote position, open to applicants in Pennsylvania.
• Execute Licensed Practical Nurse responsibilities as a patient navigator for incoming inquiries and enrollment in new patient programs.
• Educate patients and enhance their understanding of health conditions.
• Ensure ongoing continuity of care and provide clinical support to both providers and patients.
• Utilize clinical expertise to determine actions that facilitate patient integration into the program.
• Assess and document patient requirements primarily through phone interactions or review of documentation.
• Coordinate and implement patient care plans, including evaluation, treatment, and monitoring.
• Communicate with patients via mail, telephone, and face-to-face interactions.
• Organize patient needs, activities, support groups, educational sessions, and available services.
• Act as the main point of contact for providers and liaise with Scheduling Services.
• Ensure timely patient follow-up and maintain surveillance systems and processes.
• Create and implement patient screening tools, clinic protocols, and navigational procedures.
• Address patient concerns related to appointment access, communication, and billing issues.
• Oversee clinical supply inventory and maintain examination and treatment rooms.
• Conduct reviews of medical records and communicate findings to physician specialists.
• Provide nursing care and assist physicians during procedures.
• Inspect equipment and identify any preventive or corrective maintenance requirements.
• A minimum of 2 years of previous experience as an LPN is required.
• Graduation from a Specialty Training Program in Nursing is mandatory.
• A valid Licensed Practical Nurse license is required.
• Strong computer skills are essential.
• Proficient in Microsoft Word, Excel, PowerPoint, SharePoint, Windows, and Teams.
• Experience in Clinic and Pediatrics is preferred.
• Ability to work day/evening shifts between 7:00 a.m. and 11:30 p.m.
• Willingness to work every other weekend.
• Availability to work at least one major holiday and one minor holiday each year.
• Travel may be necessary for orientation/training, meetings, or other business needs (less than 25%).
• Strong communication, computer literacy, customer service, multitasking, and teamwork skills are required.
