Licensed Practical Nurse, MN Licensure – Day Shift

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Collaborate extensively with providers to implement value-based care management initiatives.

• Oversee patient census with a focus on resolving clinical and non-clinical care gaps.

• Identify and connect patients with community resources.

• Offer education and enhance health literacy related to chronic-condition management.

• Conduct medication management tasks, including recognizing medication issues, reconciliation, adherence, and coordinating refills.

• Ensure the timely provision of Home Health, DME, Home Infusion, and other essential services.

• Address patient inquiries and maintain open communication to grasp their needs.

• Assist with or oversee referrals and appointment scheduling.

⛳️ Requirements

• Minimum of 3 years of direct patient-facing experience.

• Valid Minnesota LPN license.

• Proven experience in identifying resources and coordinating needs for patients in chronic care management.

• Proficiency with Microsoft Office Suite.

• Strong written and verbal communication abilities.

• Reliable high-speed Wi-Fi connection.

• Home office or workspace that complies with HIPAA regulations.

• Capacity to sit for extended periods.

• Ability to communicate clearly and listen attentively over the phone for long durations.

• Competence in understanding, processing, and taking comprehensive notes during real-time telephone conversations.

• Experience with Epic software is preferred.

• Bilingual skills are preferred.

• Additional state LPN licenses are preferred.
