Licensed Practical Nurse, MN Licensure – Day Shift

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Oversee patient census with a focus on resolving issues to enhance both clinical and non-clinical patient care.

• Identify and coordinate community resources for patients to bolster their care.

• Deliver patient education and promote health literacy regarding chronic-condition management.

• Conduct medication management, which includes identifying medication issues, performing reconciliation, ensuring adherence, and coordinating refills.

• Aid in the timely provision of Home Health, DME, Home Infusion, and other essential services.

• Address patient inquiries and foster open communication to better understand their needs.

• Assist with or oversee referrals and appointment scheduling.

• Collaborate comprehensively with providers to implement value-based care management initiatives.

⛳️ Requirements

• Minimum of 3 years of direct patient-facing experience.

• Current Minnesota License (LPN).

• Proven experience in identifying resources and coordinating needs for patients with chronic care management.

• Technical proficiency with Microsoft Office Suite.

• Exceptional written and verbal communication skills.

• Reliable high-speed Wi-Fi connection.

• Home office or a workspace that complies with HIPAA regulations.

• Capability to sit for extended periods.

• Ability to communicate clearly and listen attentively, often by telephone, for prolonged durations.

• Proficiency in understanding, processing, and taking detailed notes in real time during phone conversations.

• Experience with Epic is an advantage.

• Bilingual skills are a plus.

• Additional state LPN licenses are an advantage.
