Licensed Practical Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in accordance with the Nurse Practice Act of the state.

• Execute and adhere to personalized care plans set forth by the RN.

• Provide education to patients and their families regarding self-care techniques and information related to diagnoses.

• Ensure accurate and timely documentation of care provided.

• Work collaboratively with the care team, which includes OT, PT, HHA, and other disciplines, while communicating patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, following aseptic techniques.

• Carry out additional responsibilities as needed for patient services.

⛳️ Requirements

• Completion of an accredited practical or vocational nursing program.

• Current LPN or LVN license in California.

• Over 3 years of nursing experience, preferably in home health or med/surg settings.

• Valid CPR certification from AHA.

• Possession of a valid driver’s license, reliable insured vehicle, and the ability to travel to patient residences.

• Excellent communication skills alongside a collaborative, patient-focused approach.
