Licensed Practical Nurse

📋 Description

• Perform comprehensive patient evaluations using telehealth platforms to assess health conditions and determine care requirements.

• Create and execute personalized care strategies in collaboration with patients and healthcare teams.

• Track patient advancement and responses to treatment, modifying care plans as required.

• Educate patients on managing diseases, adhering to medication regimens, and making healthy lifestyle choices.

• Record patient interactions, evaluations, and care strategies in electronic health records.

• Collaborate with physicians, specialists, and other healthcare professionals.

• Address patient inquiries and concerns, providing remote guidance and assistance.

• Utilize telehealth technology for virtual appointments and remote patient monitoring.

• Provide virtual nursing care to manage chronic conditions and acute health issues.

• Aid in decreasing hospital readmissions and emergency visits through proactive measures and timely communication.

⛳️ Requirements

• A current and valid Licensed Practical Nurse license in the United States.

• A license in a compact state, with a preference for additional licenses in California and Illinois.

• At least one year of clinical nursing experience, ideally in outpatient, community health, or telehealth environments.

• Basic proficiency in electronic health record (EHR) systems and telehealth technologies.

• Excellent communication skills for effective interaction with patients and healthcare team members remotely.

• Ability to work autonomously in a remote setting while effectively managing time and priorities.

• Preferred: experience in remote patient monitoring or telehealth nursing.

• Preferred: certification in telehealth nursing or a related specialty.

• Knowledge of chronic disease management programs such as diabetes, hypertension, or COPD.

• Preferred: advanced clinical training or certifications, such as Certified Case Manager (CCM) or Certified Diabetes Educator (CDE).

• Proven ability to utilize data analytics tools to monitor patient outcomes and enhance care delivery.
