Licensed Practical Nurse

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver direct patient care in compliance with the state's Nurse Practice Act.

• Execute and adhere to personalized care plans set forth by the RN.

• Inform patients and their families about self-care practices and diagnosis-related information.

• Record patient care accurately and promptly.

• Work collaboratively with the care team, including OT, PT, HHA, and other disciplines, while conveying patient needs to the RN or Clinical Supervisor.

• Utilize supplies and equipment safely, maintaining aseptic technique.

• Perform additional duties as necessary to support patient services.

⛳️ Requirements

• Completion of an accredited practical or vocational nursing program.

• Current LPN or LVN license in California.

• Minimum of 3 years of nursing experience; background in home health or med/surg preferred.

• Valid CPR certification from AHA.

• Possession of a valid driver's license, reliable insured vehicle, and capability to travel to patient residences.

• Excellent communication skills coupled with a collaborative, patient-centered approach.
