Licensed Practical Nurse – Advanced Therapeutics

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Perform telephonic and video-based patient triage.

• Conduct structured assessments of side effects and escalate observations to the relevant clinical personnel.

• Execute nurse visits concentrating on education regarding medication administration, including injectables, as well as lifestyle and behavioral assessments.

• Offer patient education regarding medications, strategies for side-effect mitigation, lifestyle changes, and program prerequisites.

• Assist in pre-initiation processes, which include baseline assessments, laboratory coordination, and collaboration with pharmacies and providers.

• Engage in continuous patient monitoring, focusing on safety labs, refill schedules, and insurance or authorization needs.

• Address patient inquiries through telephone, secure messaging, and other approved communication channels while adhering to service-level timeframes.

• Facilitate care coordination and relay patient updates to clinical, administrative, pharmacy, payer, and external provider stakeholders.

• Accurately and promptly document clinical interactions, assessments, and communications in the electronic medical record (EMR).

• Participate in team meetings, case discussions, quality improvement projects, and virtual consultations.

• Ensure a secure and private workstation environment.

⛳️ Requirements

• Possession of a current and active LPN license.

• At least 2 years of clinical experience.

• Excellent communication and patient education abilities.

• Capability to operate independently in a remote setting.

• Proficiency in telehealth technologies and remote patient communication.

• Experience in managing patients with chronic diseases is preferred.

• Familiarity with clinical patient triage and symptom assessment evaluation is preferred.

• Knowledge of electronic medical record systems, preferably Epic, is preferred.

• Ability to assist patients with specialty disease states such as Dermatology, Inflammatory Bowel Disease (IBD), and Rheumatology.
