Licensed Clinical Social Worker

This is a fully remote position, open to applicants in Florida.

📋 Description

• Oversee a caseload of high-risk patients with socially complex needs.

• Perform psychosocial assessments through in-person meetings and phone calls with patients and/or their caregivers.

• Examine psychological and social determinants that hinder care and create interventions to address these gaps.

• Offer short-term counseling services as necessary.

• Coordinate services to ensure that patients and families comprehend their treatment plans.

• Work in collaboration with the care team to meet quality and patient experience goals, including active involvement in High-Risk Huddles.

• Reach out to community-based services that support patients' ability to age in place.

• Establish and implement shared care goals with patients and their families.

• Partner with community organizations to build relationships and offer resources that promote patient independence.

• Collaborate with providers, support staff, patients, caregivers, facilities, third-party organizations, external providers, and community resources.

⛳️ Requirements

• Master’s degree in social work.

• Must hold a valid LCSW or LISW license (depending on state regulations).

• Must possess a valid driver's license and maintain a good driving record.

• Ability to exercise sound clinical judgment and communicate effectively in both written and verbal forms.

• Proficient computer skills.

• Capacity to work independently and communicate effectively with professional staff across various disciplines and programs.

• Ability to manage time efficiently, juggle multiple tasks, work within tight deadlines, resolve issues, identify patient service trends, recognize system improvements, and implement necessary changes.

• Capability to share knowledge with others while understanding the importance of fostering performance improvement to achieve patient satisfaction and operational efficiency.

• Experience in Care Management within a community-based environment or similar (preferred).

• Experience working with patients who have chronic, complex, and/or behavioral health issues (preferred).
