Kidney Palliative Care Social Worker

This is a fully remote position, open to applicants in California.

📋 Description

• Assist patients and their families in navigating chronic kidney disease, end-stage kidney disease, and end-of-life care.

• Lead discussions regarding Goals of Care.

• Facilitate Advance Care Planning discussions.

• Offer support for anticipatory grief.

• Address concerns related to behavioral health.

• Provide remote virtual support for patients.

• Conduct assessments and interventions in palliative social work and behavioral health.

• Support patients with advanced kidney disease and their caregivers.

• Educate patients, families, and caregivers on available community resources and palliative and hospice services.

• Advocate for the needs and preferences of patients.

• Collaborate with interdisciplinary teams to create and implement personalized care plans.

• Serve as a liaison with dialysis clinic staff, nephrology practices, and external providers to ensure goal-concordant care.

⛳️ Requirements

• Possession of a current and active LCSW license in California.

• Must reside in the Pacific Standard Time (PST) or Mountain Standard Time (MST) zones.

• Master’s degree in Social Work from an accredited institution.

• A minimum of one year of experience in hospice or palliative care.

• At least 6–8 years of social work experience, ideally within healthcare environments.

• Exceptional interpersonal and communication abilities.

• Capability to deliver quality patient care in a remote work environment.

• Strong assessment skills.

• Knowledge of palliative care principles.

• Familiarity with grief counseling methods.

• Understanding of advance care planning processes.

• Insight into behavioral health interventions.

• An empathetic, self-motivated, and flexible approach.

• Proficiency in managing emotionally charged situations with professionalism.
