This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver comprehensive care services to patients with End-Stage Renal Disease (ESRD)

• Collaborate with patients to avert complications

• Aid in minimizing preventable hospital admissions

• Enhance the overall health of patients

• Align with DaVita Integrated Kidney Care’s national vision for integrated care

• Play a key role in the care of medically complex and vulnerable patient populations
