
Intake Associate, Weekend Hospice/Palliative

This is a fully remote position, open to applicants in United States.
• Receive and manage hospice and palliative care referrals from physicians, hospitals, facilities, and community sources.
• Collect accurate patient medical and environmental details to initiate hospice and palliative consultations.
• Relay patient priorities and referral details to hospice and palliative care teams.
• Accurately process referrals within established timelines.
• Schedule, check in, and confirm face-to-face visits with Nurse Practitioners.
• Coordinate initial evaluations involving patients, families, and Nurse Practitioners.
• Draft denial letters and assist with one-time patient contracts.
• Maintain the on-call schedule for the Hospice Administrator.
• Register new hospice patients with the Hospice Pharmacy.
• Support the creation of hospice and palliative care EMR records.
• Review PMP reports for new hospice admissions and forward them to physicians for evaluation.
• Handle forms, signatures, documentation, insurance verification, and admission data entry.
• Respond to telephone inquiries and provide interpreting services to callers.
• Utilize Email, Epic EMR, and other information technologies to manage and track referrals.
• Build and sustain relationships with referral sources, patients, families, and staff.
• Participate in meetings, in-service programs, and quality improvement initiatives.
• Assist in mentoring new staff, preparing for IDT, and tracking face-to-face visits.
• Uphold confidentiality and adhere to organizational policies, HIPAA regulations, and mandatory education requirements.
• High school diploma is required.
• 1-2 years of experience in a health-related field.
• Exceptional observation and problem-solving/critical thinking abilities.
• Strong verbal and written communication skills.
• Essential computer skills for working with EMR.
• Proficient in Excel.
• Must possess a valid driver's license and an insured automobile that meets state or organizational standards and is in good condition.
• Self-motivated and capable of working with minimal supervision.
• Strong organizational and workload prioritization skills independently.
• Proficient with common office software and able to learn Epic and E-Discharge.
• Ability to maintain confidentiality and comply with HIPAA regulations.
• Capability to complete required training and mandatory education.
• An Associate’s degree in a healthcare-related field or business is preferred.
• Influenza vaccination is required as a condition of employment.
• Potential for shift differentials, call pay, premium pay, and overtime compensation.
