This is a fully remote position, open to applicants in Canada.
• Offer assistance to patients and healthcare providers concerning Patient Support services, diseases, and treatment options.
• Aid patients and caregivers with assessments related to Patient Support services.
• Serve as a liaison between patients, families, and healthcare providers regarding medication accessibility.
• Respond to inquiries regarding program services, disease states, and products.
• Provide continuous disease-state and product support to enhance patient adherence.
• Refer patients to healthcare providers for clinical inquiries beyond the scope of program materials.
• Record patient information and interactions in electronic patient records.
• Conduct ongoing file reviews and update patient journeys accordingly.
• Adhere to case-management protocols, including data collection, follow-up, monitoring therapy interruptions/discontinuations, reporting adverse events, and liaising with primary healthcare providers.
• Collaborate with reimbursement specialists and physicians to secure funding for therapy and explore alternatives.
• Coordinate and monitor patient services according to KPIs to facilitate therapy access.
• Participate in specialized therapy training sessions.
• Attend pharmaceutical meetings, patient gatherings, conferences, and engage with patient associations and advocacy groups.
• Coordinate logistics and issue resolution with healthcare providers, pharmacies, and infusion/injection clinics.
• Maintain communication with the manager, team, and client regarding patient cases.
• Support the training of new Nurse Case Managers and provide feedback on protocols, databases, processes, and interdepartmental workflows.
• Report adverse events and serious adverse events following program procedures.
• Undertake other assigned duties and tasks as needed.
• May be required to provide standby/on-call coverage.
• A Diploma in healthcare or a related field; a Bachelor’s degree is preferred.
• An active license as a Nurse (RN or RPN/LPN) with registration from the College of Nurses in the province of employment is required.
• A minimum of two (2) to five (5) years of experience in healthcare, pharmaceuticals, insurance, or related fields.
• Ideal candidates should possess nursing experience with a background in healthcare and case management within medical/hospital settings, insurance, or the bio-pharmaceutical industry.
• Fluency in French and/or English may be required based on program needs and geographic location.
• Experience in healthcare and case management is essential.
• Strong problem-solving skills and the ability to work independently as well as collaboratively within a team.
• Excellent analytical and problem-solving capabilities.
• Capacity to work effectively across functions and within a small team environment.
