Field Care Coordinator – Bilingual Preferred

This is a fully remote position, open to applicants in Florida.

📋 Description

• Evaluate and assess the needs and requirements of Medicaid members.

• Conduct visits to members in their homes, assisted living facilities, long-term care facilities, and various care settings.

• Coordinate services and resources to promote members' optimal wellness.

• Develop individualized care plans specific to each member.

• Ensure that members receive services in the least restrictive environments possible.

• Plan and execute interventions as needed.

• Monitor and assess case management plans in relation to members' personal goals.

• Guide members and their families towards suitable care resources.

• Facilitate communication with payers, providers, interdisciplinary teams, and others involved in members' care.

• Identify and address barriers to effective care delivery.

• Continuously oversee patient care via assessments, data analysis, conversations, and active care planning.

• Travel locally 75–90% of the time.

• Report directly to the Regional Care Coach Manager.

⛳️ Requirements

• Must reside in Collier County, Florida.

• Bachelor's degree in the field of health and human services.

• A minimum of 2 years of relevant experience.

• Outstanding verbal and written communication skills.

• Proficiency in various electronic information applications and software programs.

• Possession of a valid state driver's license.

• Access to a reliable vehicle.

• Proof of personal vehicle liability insurance with minimum limits of 100,000/300,000/100,000.

• Must comply with Florida Level 2 background screening requirements through the Care Provider Background Screening Clearinghouse.

• Minimum home internet download speed of 25 Mbps and upload speed of 10 Mbps.

• Ability to work from a designated area without ongoing interruptions to safeguard member PHI/HIPAA information.
