In-Home Traveler Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Hawaii.

📋 Description

• Conduct health evaluations in the homes of members.

• Establish trustworthy relationships to enhance health outcomes.

• Perform approximately 45-minute visits that include reviewing medication, medical history, and conducting physical evaluations.

• Execute additional diagnostic tests as requested by the individual's health plan, such as diabetic eye exams and spirometry.

• Utilize an iPad and clinical workflow for every evaluation.

• Travel nationwide to connect individuals with suitable care.

• Travel locally and within licensed states, covering all counties and additional areas as necessary.

• Conduct virtual visits when required.

• Deliver Signify Health services, including Diagnostic & Preventive Services like spirometry, DEE, and bone density screening.

• Primarily work in the private residences of health plan members.

⛳️ Requirements

• Possess an active, unrestricted license in the relevant coverage areas.

• Board certification is mandatory.

• Ability to acquire and maintain multi-state licenses as necessary.

• Capable of covering all counties within the state and adaptable to other business needs.

• Comfortable performing duties in homes that may include pets, secondhand smoke, and other airborne allergens or irritants.

• Willingness to travel up to 100% of the time, which may involve overnight hotel stays, car rentals, and/or flights.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Competence in conducting virtual visits when required.
