In-Home Physician, Full Time

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Conduct in-home health assessments within members' residences.

• Travel extensively, potentially up to 100% of the time, within a designated coverage area.

• Perform visits lasting around 45 minutes while establishing reliable, one-on-one relationships.

• Assess medications and review medical histories.

• Execute physical evaluations.

• Administer additional diagnostic tests as needed, such as diabetic eye exams, spirometry, and bone density screenings.

• Utilize an iPad and user-friendly clinical workflow for each evaluation.

• Conduct virtual visits when necessary.

• Deliver Signify Health services, including Diagnostic & Preventive Services offerings.

• Engage with individuals nationwide and link them to suitable care options.

• Make referrals to health plans and promote family involvement.

• Inform primary care physicians about patients' health status and quality of life.

• Collaborate as a clinician colleague with Signify Health.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage area(s).

• Capable of obtaining and maintaining multi-state licenses in the necessary coverage area.

• Must be a Physician (MD or DO).

• Comfortable and safe performing duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Willingness to travel locally and within licensed states.

• Ability to conduct virtual visits when required.

• Proficient in using an iPad and navigating clinical workflows.

• Expected availability of 40 hours per week.
