In-Home Nurse Practitioner, Physician Assistant – Per Diem

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Conduct in-home health evaluations lasting approximately 45 minutes.

• Interact individually with health plan members, addressing their health inquiries.

• Examine medications and review medical history.

• Perform physical assessments.

• Execute additional diagnostic tests as ordered by the health plan, which may include diabetic eye examinations and spirometry.

• Establish trusted relationships to enhance health outcomes.

• Travel nationwide to connect individuals with suitable care.

• Meet the requirements of the designated clinician position with Signify Health.

• Travel locally and within licensed states as necessary.

• Conduct virtual visits when required.

• Deliver Diagnostic & Preventive Services, encompassing spirometry, diabetic eye exams, and bone density screenings.

⛳️ Requirements

• Possess an active, unrestricted license(s) within coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Qualifications as a Nurse Practitioner or Physician Assistant.

• Willingness to travel locally and within licensed states as needed.

• Capability to perform visits in private homes that may have household pets, secondhand smoke, and airborne allergens or irritants.

• Proficient in using an iPad and navigating intuitive clinical workflows.

• Expected weekly hours: 12.
