In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Conduct In-Home Health Evaluations in the residences of members

• Carry out visits lasting approximately 45 minutes

• Interact one-on-one with members and address their health inquiries

• Assess medications and review medical history

• Execute physical evaluations

• Carry out additional diagnostic tests as prescribed by the health plan, such as diabetic eye exams and spirometry

• Utilize an iPad and user-friendly clinical workflow for each assessment

• Travel nationwide to link individuals with suitable care

• Travel locally and within licensed states as necessary

• Conduct virtual visits when needed

• Offer Diagnostic & Preventive Services, including spirometry, diabetic eye exams, and bone density screenings

• Establish trusted relationships to enhance health outcomes

• Active and unrestricted license(s) in the applicable coverage area(s)

• Board certification (mandatory)

• Multi-state licenses (preferred)

• Willingness to travel locally and within licensed states as required

• Capability to conduct virtual visits as necessary

• Ability to safely perform duties in homes that may include household pets, secondhand smoke, and other potential airborne allergens or irritants

• Qualification as a Nurse Practitioner or Physician Assistant

• Availability for full-time work, with 40 expected hours per week
