In-Home NP/PA – Part Time

This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Conduct part-time in-home health evaluations at the residences of members.

• Interact with individuals to help close care gaps.

• Perform visits lasting around 45 minutes each.

• Assess medications and review medical histories.

• Carry out physical assessments.

• Administer additional diagnostic tests as requested by the health plan, including diabetic eye exams and spirometry.

• Utilize an iPad and user-friendly clinical workflow during evaluations.

• Establish trustworthy relationships to enhance health outcomes.

• Visit health plan members across the country and link them to suitable care.

• Travel locally and within licensed states as required.

• Offer virtual visits when necessary.

• Provide Signify Health Diagnostic & Preventive Services, such as spirometry, diabetic eye exams, and bone density screenings.

• Mainly operate in the private homes of health plan members.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Qualification as a Nurse Practitioner or Physician Assistant.

• Willingness to travel locally and within licensed states as necessary.

• Capability to conduct visits safely in private homes that may have pets, secondhand smoke, or airborne allergens and irritants.

• Proficient in using an iPad and clinical workflows for evaluations.

• Competence in performing medication and medical history reviews, physical evaluations, and diagnostic and preventive services.

• Expected weekly hours: 29.5.
