In Home NP/PA – Full Time

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct in-home health assessments at the residences of members

• Execute visits lasting approximately 45 minutes, which include reviewing medication and medical history as well as performing physical evaluations

• Carry out additional diagnostic tests as directed by the health plan, such as diabetic eye exams and spirometry

• Utilize an iPad and follow clinical workflows for each evaluation

• Establish trusted relationships to enhance health outcomes

• Travel locally and within permitted states as necessary

• Conduct virtual consultations when required

• Deliver Signify Health services, which include Diagnostic & Preventive Services like spirometry, DEE, and bone density screenings

⛳️ Requirements

• Active and unrestricted license(s) in relevant coverage areas

• Board certification (mandatory)

• Multi-state licenses (preferred)

• Capability to safely and comfortably perform tasks in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants

• Qualification as a Nurse Practitioner or Physician Assistant
