Improvement Advisor

This is a fully remote position, open to applicants in Utah.

📋 Description

• Assist organizations in enhancing quality and executing community health initiatives.

• Mentor partners in data gathering, analysis, and strategies for performance improvement.

• Create and execute evidence-driven quality enhancement interventions.

• Aid in the development and refinement of care coordination and referral processes.

• Lead learning collaboratives, training sessions, technical support, and partner engagement meetings.

• Collaborate with interdisciplinary teams and Community Health Workers to bolster care coordination, community involvement, referral processes, and community health initiatives.

• Facilitate partner readiness, onboarding, and implementation efforts with healthcare and community-based organizations.

• Monitor and record activities utilizing CRM and project management tools.

• Offer consulting and technical support to hospitals, practices, local health departments, and community-based organizations.

• Assist in the planning, execution, and operational management of the Utah Community Care Hub.

⛳️ Requirements

• Bachelor’s degree in public health, healthcare, or a related discipline.

• Over 4 years of experience in healthcare, public health, community-based social services, or a related field, including quality improvement work.

• At least 2 years of project management experience.

• Strong understanding of quality improvement techniques, healthcare terminology, and clinical workflows.

• Exceptional communication and partner engagement abilities.

• Prior experience in an outpatient clinic is preferred.

• A Master’s degree (MA/MS) in Public Health, Nursing, Health Care Administration, Public Policy, or a related field is preferred.
