I/DD Care Manager – Mobile

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Act as the primary liaison and guide for members with Intellectual and Developmental Disabilities (I/DD) and/or dual diagnoses.

• Lead a Team-Based Care approach that integrates physical health, behavioral health, and long-term support services.

• Meet with members in their local communities to evaluate current and anticipated needs.

• Develop Person-Centered Care Plans and Individual Support Plans (ISP).

• Facilitate interdisciplinary team meetings involving doctors, specialists, providers, and families.

• Assist members through transitions such as moving from school to adulthood and reintegrating from care facilities.

• Support members in achieving optimal independence and locating suitable paid supports.

• Educate members and their families about their rights.

• Connect members with services and the provider network.

• Travel frequently within assigned communities to fulfill job responsibilities.


⛳️ Requirements

• Bachelor’s degree in a human services discipline and a minimum of 2 years of full-time experience working with individuals with Intellectual and Developmental Disabilities, including at least 2 years in a community care setting with significant long-term support needs.

• Alternatively, a bachelor’s degree in a field other than human services, coupled with at least 4 years of full-time experience working with individuals with Intellectual and Developmental Disabilities, including at least 2 years in a community care setting with significant long-term support needs.

• Or a master’s degree along with a minimum of 1 year of full-time experience working with individuals with Intellectual and Developmental Disabilities, including at least 2 years in a community care setting with significant long-term support needs.

• Residency in North Carolina.

• Capability to travel regularly as required to perform job duties.

• Understanding of, or a willingness to learn, Medicaid regulations, 1915i services, and the Tailored Plan landscape.

• Skills in person-centered planning.

• Ability to work in a mobile, community-oriented role.

• Experience in care management with individuals with Intellectual and Developmental Disabilities.
