This is a fully remote position, open to applicants in United States.
• Evaluate clinical documentation for home health and hospice at various time points, including SOC, ROC, Recertification, Transfer, Discharge, and Admission.
• Accurately assign ICD-10-CM diagnosis codes.
• Sequence diagnoses following ICD-10-CM guidelines, CMS requirements, PDGM payment methodologies, and hospice coding standards.
• Conduct audits of clinical charts to identify documentation gaps or inconsistencies.
• Work in partnership with clinical leadership and compliance teams to address query requirements.
• Achieve productivity and turnaround-time targets to facilitate billing and claims processing.
• Provide assistance with internal quality assurance and external medical reviews, including TPE, ADRs, and compliance audits.
• Collaborate with clinical, QA, compliance, and revenue cycle teams to enhance documentation integrity and regulatory preparedness.
• Stay updated on CMS guidance, Medicare regulations, LCDs, and payer-specific changes.
• Safeguard PHI and ensure adherence to HIPAA regulations.
• Create and maintain tracking logs, status updates, and coding reports for management.
• Minimum of 2 years of experience in Home Health, ICD-10 coding, and OASIS review.
• Current HCS-D and HCS-O certifications are required.
• Extensive experience in reviewing Plan of Care (POC) and HOPE documentation.
• In-depth knowledge of PDGM, Medicare CoPs, HHVBP, Star Ratings, and ICD-10 Official Coding Guidelines.
• Strong understanding of home health and hospice coding guidelines, Medicare Conditions of Participation, LCD/NCD guidance, PDGM requirements, hospice eligibility documentation, and diagnosis sequencing.
• CHS-C certification is preferred.
• Experience in dual-specialty coding within Home Health and Hospice care environments.
• Proven experience in supporting TPE, ADR, medical reviews, and internal/external compliance audits.
• Proficiency with industry-standard EMRs, including Homecare Homebase, WellSky, MatrixCare, Careficient, and Netsmart.
• Solid grasp of OASIS accuracy, clinical quality measures, hospice documentation integrity, and billing-related coding standards.
• Excellent verbal and written communication abilities.
• Capability to work independently, prioritize tasks, meet tight deadlines, and function with minimal supervision.
• Strong clinical judgment and keen attention to detail.
• Skilled in proactive problem identification, timely escalation, confidentiality, and HIPAA compliance.
• Ability to secure and maintain personal equipment, ensure internet access, and hold relevant certifications.
• Must reside within the United States.
• Flexible schedule with no fixed hours, subject to established review-assignment due dates.
• 1099 contractor arrangement.
