This is a fully remote position, open to applicants in Connecticut, +3 more states.
• Create a housing strategy tailored for Horizon membership, focusing specifically on MLTSS/SNP/Medicaid.
• Identify and oversee resources for various housing options.
• Educate Care Management, Member Advocates, Utilization Management, and other pertinent staff regarding available housing options and resources.
• Assist in identifying and placing members in suitable housing while facilitating transitions to community environments.
• Prepare reports to fulfill state mandates, including tracking progress towards goals and quality assurance monitoring.
• Serve as a liaison with DMAHS personnel or their designee to receive training and support for capacity building.
• Collaborate closely with public and private housing resources and stakeholders, which include HUD-subsidized housing, DCA, NJ HMFA voucher programs, public housing authorities, realtors, and online housing locator tools.
• High School Diploma or GED is mandatory.
• A minimum of three (3) years of full-time experience assisting vulnerable populations (such as the homeless, elderly, and individuals with disabilities) in securing accessible and affordable housing.
• At least two (2) years of experience working for a public agency or in a related social service or community service role.
• Familiarity with relevant public and private housing resources and stakeholders, including HUD-subsidized housing, the Department of Community Affairs (DCA), New Jersey Housing and Mortgage Finance Agency (NJ HMFA) housing program voucher initiatives, public housing authorities, realtors, and online housing locator tools.
• Knowledge of Medicare and Medicaid is essential.
• Understanding of Managed Long-Term Services and Supports (MLTSS).
• Strong interpersonal and advocacy skills are required.
• Excellent verbal and written communication abilities are a must.
• Proficient PC skills and familiarity with Microsoft Office applications (Excel, Access, Word) are necessary.
• Strong problem-solving, investigative, mediation, and conflict resolution skills are required.
• Competence in reporting and delivering oral presentations is essential.
• Strong organizational skills and the ability to work with minimal daily supervision are essential.
• Ability to exercise sound judgment and maintain a professional demeanor.
• A solid background in processing prior authorizations and proven expertise in navigating various systems, including care management systems, HMIS, and the Medicaid Eligibility Verification System, is required.
• A valid New Jersey State driver’s license and insurance are mandatory.
