House Calls Nurse Practitioner

This is a fully remote position, open to applicants in Nebraska, +5 more states.

📋 Description

• Conduct telehealth consultations for patients participating in the House Calls program.

• Manage chronic health conditions including diabetes, hypertension, CHF, COPD, and geriatric syndromes when primary care services are necessary.

• Perform assessments of care management needs, conduct follow-ups, and make adjustments to care plans.

• Review patient medical histories, laboratory results, imaging, and diagnostic information.

• Assist with medication management and reconciliation.

• Coordinate care with primary care providers, specialists, and care management teams.

• Accurately document visits and clinical decisions in the electronic medical record (EMR).

• Ensure adherence to clinical protocols, quality standards, and regulatory guidelines.

• Participate in interdisciplinary case reviews and initiatives aimed at quality improvement.

• Provide virtual clinical oversight, conduct patient assessments, coordinate care, and support providers for patients receiving care at home.

⛳️ Requirements

• Active Nurse Practitioner (NP) license in the required state(s).

• National NP certification (ANCC or AANP).

• At least 2 years of clinical experience as a Nurse Practitioner.

• Preference for candidates with experience in house calls, home health, geriatrics, primary care, or internal medicine.

• Prior experience in telehealth or remote care is preferred.

• Strong clinical judgment and a high level of comfort in managing complex patients remotely.

• Exceptional communication and documentation skills.

• Must hold a license in NY and one additional state listed, or be willing to obtain one.

• Proficient with EMR/EHR systems.

• Comfortable using telehealth platforms and remote monitoring technologies.

• Active DEA registration (or willingness to obtain, if applicable).
