House Calls Nurse Practitioner

This is a fully remote position, open to applicants in Nebraska, +5 more states.

📋 Description

• Conduct virtual health consultations for patients participating in the House Calls program.

• Oversee chronic conditions like diabetes, hypertension, CHF, COPD, and geriatric syndromes when primary care services are needed.

• Execute care management assessments, follow-ups, and adjustments to care plans.

• Analyze patient histories, lab results, imaging, and diagnostic outcomes.

• Assist with medication management and reconciliation processes.

• Facilitate coordination of care with primary healthcare providers, specialists, and care management teams.

• Accurately document visits and clinical decisions in the EMR system.

• Ensure adherence to clinical protocols, quality standards, and regulatory guidelines.

• Engage in interdisciplinary case reviews and quality enhancement initiatives.

⛳️ Requirements

• Active Nurse Practitioner (NP) license in the necessary state(s).

• National NP certification (ANCC or AANP).

• A minimum of 2+ years of clinical experience as a Nurse Practitioner.

• Experience in house calls, home health, geriatrics, primary care, or internal medicine is highly preferred.

• Previous telehealth or remote care experience is preferred.

• Strong clinical judgment and the ability to manage complex patients remotely with confidence.

• Outstanding communication and documentation abilities.

• Must hold a license in NY and one of the other specified states or be willing to obtain it.

• Proficient in using EMR/EHR systems.

• Comfortable with telehealth platforms and remote monitoring tools.

• Active DEA registration (or willingness to obtain, if applicable).
