House Calls Nurse Practitioner

This is a fully remote position, open to applicants in Nebraska, +5 more states.

📋 Description

• Conduct telehealth appointments for patients participating in the House Calls program.

• Manage chronic illnesses such as diabetes, hypertension, CHF, COPD, and geriatric syndromes when the patient needs primary care services.

• Perform assessments for care management needs, follow up, and adjust care plans as necessary.

• Review patient histories, laboratory results, imaging, and diagnostic findings.

• Assist in medication management and reconciliation.

• Coordinate care with primary care providers, specialists, and care management teams.

• Accurately document visits and clinical decisions in the electronic medical record (EMR).

• Ensure adherence to clinical protocols, quality standards, and regulatory guidelines.

• Engage in interdisciplinary case reviews and initiatives for quality improvement.

• Provide virtual clinical oversight, patient evaluations, care coordination, and support for providers assisting patients receiving care at home.

⛳️ Requirements

• Current Nurse Practitioner (NP) license in the necessary state(s).

• National NP certification (ANCC or AANP).

• At least 2+ years of clinical experience as a Nurse Practitioner.

• Experience in house calls, home health, geriatrics, primary care, or internal medicine is highly preferred.

• Previous experience in telehealth or remote care is preferred.

• Strong clinical judgment and confidence in managing complex patients remotely.

• Exceptional communication and documentation abilities.

• Must hold a license in NY and one of the other states listed, or be willing to obtain it.

• Proficient with EMR/EHR systems.

• Comfortable using telehealth platforms and remote monitoring technologies.

• Active DEA registration (or willingness to obtain, if applicable).
