House Calls Nurse Practitioner

This is a fully remote position, open to applicants in Nebraska, +5 more states.

📋 Description

• Conduct telehealth consultations for patients participating in the House Calls program

• Manage chronic illnesses such as diabetes, hypertension, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), and geriatric syndromes as necessary for primary care services

• Perform assessments related to care management needs, follow-ups, and adjustments to care plans

• Review patient medical histories, laboratory results, imaging studies, and diagnostic findings

• Assist with medication management and reconciliation

• Collaborate with primary care providers, specialists, and care management teams to coordinate patient care

• Accurately document visits and clinical decisions within the electronic medical record (EMR)

• Ensure adherence to clinical protocols, quality standards, and regulatory guidelines

• Engage in interdisciplinary case reviews and initiatives aimed at quality improvement

• Provide virtual clinical oversight, patient evaluations, care coordination, and support for providers caring for patients in their homes

⛳️ Requirements

• Active Nurse Practitioner (NP) license in the necessary state(s)

• National NP certification (from ANCC or AANP)

• At least 2+ years of clinical experience as a Nurse Practitioner

• Preference for experience in house calls, home health, geriatrics, primary care, or internal medicine

• Prior experience with telehealth or remote care is preferred

• Strong clinical judgment and comfort in managing complex patients remotely

• Excellent skills in communication and documentation

• Must hold a license in NY and one of the other listed states, or be willing to obtain one

• Proficient in EMR/EHR systems

• Comfortable using telehealth platforms and remote monitoring technologies

• Active DEA registration (or willingness to obtain if applicable)
