Hospice Triage RN, PT, OVN, 3 Week Rotation, 10p-5a CST

This is a fully remote position, open to applicants in Ohio.

📋 Description

• Deliver telephone triage services for hospice patients and their families.

• Evaluate patient conditions and decide on suitable next actions.

• Work in partnership with on-call teams to manage care and resources.

• Meticulously document all communications and interventions.

• Maintain a composed and professional attitude while addressing urgent calls.

• Offer essential after-hours support and coordinate care for patients receiving hospice services.

• Record calls using electronic medical records and tailored client protocols.

⛳️ Requirements

• Active multistate Registered Nurse (RN) license is required.

• Experience in hospice, palliative, or end-of-life care is strongly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Capable of typing documentation and notes in a fast-paced setting.

• Ability to manage stress and multitask while handling a minimum of 5 calls per hour on weekdays and up to 8 calls per hour on weekends.

• Excellent communication and critical thinking abilities.

• Capability to work independently in a remote setting.

• Reliable attendance, effective communication, and stable internet connectivity during scheduled shifts.

• Availability for the overnight schedule from 10 p.m. to 5 a.m. CST on a 3-week rotation.
