Hospice Triage RN, PT

This is a fully remote position, open to applicants in Virginia, +1 more state.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify the appropriate next steps for care.

• Work in conjunction with on-call teams to coordinate resources and care.

• Ensure accurate documentation of all communications and interventions.

• Maintain a composed and professional attitude while managing urgent calls.

• Offer after-hours support and coordinate care for patients receiving hospice services.

⛳️ Requirements

• Current multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for documenting calls.

• Capability to type documentation and notes in a fast-paced environment.

• Competence in managing stress and multitasking when receiving calls.

• Excellent communication and critical thinking abilities.

• Ability to work autonomously in a remote setting.

• Regular attendance, proactive communication, and reliable internet connection during scheduled shifts.

• Availability to work the designated schedule: 3:30 PM – 11 PM, including rotating Saturday and Sunday shifts from 9:30 AM – 4 PM CST.
