Hospice Triage RN, PT

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and determine suitable next steps.

• Collaborate with on-call teams to ensure coordinated care and resource allocation.

• Accurately record communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

• Provide support for after-hours hospice care and facilitate timely interventions.

⛳️ Requirements

• Must hold an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR).

• Capable of typing documentation and notes in a fast-paced setting.

• Ability to manage stress and multitask while receiving calls.

• Excellent communication and critical thinking skills.

• Capable of working independently in a remote setting.

• Must exhibit consistent attendance, active communication, and reliable internet connection during all scheduled shifts.

• Scheduled hours: 3:30 PM – 11 PM CST, including rotating shifts on Saturdays and Sundays.
