Hospice Triage RN, PT

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify suitable next steps.

• Work collaboratively with on-call teams to manage care and resources.

• Ensure accurate documentation of communications and interventions.

• Maintain a composed and professional demeanor while addressing urgent calls.

• Offer essential after-hours support and facilitate timely interventions for hospice patients and families.

⛳️ Requirements

• Current multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for documentation purposes.

• Capable of typing documentation and notes efficiently in a fast-paced environment.

• Ability to manage stress and multitask effectively while receiving calls.

• Excellent communication and critical thinking abilities.

• Ability to work independently in a remote setting.

• Consistent attendance and proactive communication during all scheduled shifts.

• Dependable internet connectivity during all scheduled shifts.

• Base pay starting at $25 per hour, with potential increases up to $28 per hour within the first 6 months of employment.
