Hospice Triage RN, PT

This is a fully remote position, open to applicants in United States.

📋 Description

• Conduct telephone triage for hospice patients and their families

• Evaluate patient conditions and identify suitable next steps

• Collaborate with on-call teams to ensure coordinated care and resource allocation

• Meticulously document communications and interventions

• Maintain a composed and professional attitude while managing urgent calls

• Provide essential after-hours support for hospice patients and family requirements via phone

• Facilitate timely interventions and care for patients utilizing hospice services

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license

• Experience in hospice, palliative, or end-of-life care is highly preferred

• Proficient with technology and electronic medical records (EMR)

• Capable of typing documentation and notes in a fast-paced setting

• Skilled in managing stress and multitasking while answering calls; minimum of 5 calls per hour on weekdays and up to 8 per hour on weekends

• Excellent communication and critical thinking abilities

• Capable of working autonomously in a remote setting

• Consistent attendance, proactive communication, and reliable internet connection during scheduled shifts
