Hospice Triage RN, PT

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver telephone triage services for hospice patients and their families.

• Evaluate patient conditions and identify appropriate next steps.

• Collaborate with on-call teams to facilitate care and resource coordination.

• Ensure accurate documentation of communications and interventions.

• Maintain a composed and professional attitude while managing urgent calls.

• Provide after-hours assistance and ensure timely interventions for hospice patients receiving care.

⛳️ Requirements

• Possession of an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for call documentation.

• Capability to swiftly document notes and communications in a fast-paced setting.

• Skill in managing stress and multitasking during calls.

• Excellent communication and critical thinking abilities.

• Ability to work autonomously in a remote environment.

• Reliable attendance, effective communication, and stable internet connectivity during all scheduled shifts.

• Availability to work the designated schedule from 3:30 p.m. to 11:00 p.m. CST, including rotating Saturday and Sunday shifts.
