Hospice Triage RN, PT

This is a fully remote position, open to applicants in Texas.

📋 Description

• Deliver telephone triage services for hospice patients and their families

• Evaluate patient conditions and decide on the appropriate next steps

• Collaborate with on-call teams to facilitate care and resource coordination

• Precisely document all communications and interventions

• Maintain a composed and professional attitude while managing urgent calls

• Provide essential after-hours support and ensure timely interventions for hospice services

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license

• Experience in hospice, palliative, or end-of-life care is highly preferred

• Proficient with technology and electronic medical records (EMR) for call documentation

• Capable of typing documentation and notes in a dynamic environment

• Ability to manage stress and multitask while taking calls

• Excellent communication and critical thinking abilities

• Capacity to work independently in a remote setting

• Regular attendance, effective communication, and reliable internet connection during all scheduled shifts

• Availability from 3:30 p.m. to 11:00 p.m. CST, including rotating shifts on Saturdays and Sundays
