Hospice Triage RN, PT

This is a fully remote position, open to applicants in Tennessee.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and establish appropriate next steps.

• Work together with on-call teams to synchronize care and resources.

• Precisely document all interactions and interventions.

• Maintain a composed and professional attitude while managing urgent calls.

• Offer essential after-hours support for the needs of hospice patients and their families.

• Facilitate timely interventions and care for patients enrolled in hospice services.

⛳️ Requirements

• Possession of an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Capability to type notes and documentation in a fast-paced setting.

• Ability to manage stress and multitask while receiving calls.

• Strong communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• Consistent attendance, proactive communication, and dependable internet connectivity during all scheduled shifts.
