Hospice Triage RN, PT

This is a fully remote position, open to applicants in South Carolina.

📋 Description

• Deliver telephone triage for hospice patients and their families.

• Evaluate patient conditions and determine suitable next steps.

• Collaborate with on-call teams to facilitate care and allocate resources.

• Precisely document all communications and interventions.

• Maintain a calm and professional demeanor while managing urgent calls.

• Provide essential after-hours support and coordinate timely interventions for hospice services.

⛳️ Requirements

• Active multistate Registered Nurse (RN) license is required.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Ability to type documentation and notes efficiently in a fast-paced setting.

• Capability to manage stress and multitask while receiving calls.

• Strong communication skills and critical thinking abilities.

• Ability to work autonomously in a remote environment.

• Consistent attendance, proactive communication, and reliable internet connectivity during all scheduled shifts.

• Scheduled availability: 3:30 PM – 9 PM plus rotating Saturday and Sunday shifts, 9:30 AM – 4 PM CST.
