Hospice Triage RN, PT

This is a fully remote position, open to applicants in North Carolina.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify suitable next steps.

• Collaborate with on-call teams to facilitate care and resources.

• Ensure accurate documentation of all communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

• Deliver essential after-hours support and coordinate timely interventions for hospice patients receiving services.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Capability to type documentation and notes in a fast-paced setting.

• Competence in managing stress and multitasking while receiving calls.

• Excellent communication and critical thinking abilities.

• Ability to work independently in a remote setting.

• Reliable attendance, proactive communication, and consistent internet connectivity during all scheduled shifts.
