Hospice Triage RN, PT

This is a fully remote position, open to applicants in Missouri.

📋 Description

• Provide telephone triage for patients and families in hospice care.

• Evaluate patient conditions to determine the most suitable next steps.

• Collaborate with on-call teams to ensure seamless care and resource coordination.

• Accurately document all communications and interventions.

• Maintain a composed and professional demeanor while addressing urgent calls.

• Offer essential after-hours support for the needs of hospice patients and their families.

• Ensure timely interventions and care coordination for patients receiving hospice services.

⛳️ Requirements

• Must hold an active multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficient with technology and electronic medical records (EMR) for call documentation.

• Ability to type documentation and notes efficiently in a fast-paced environment.

• Capability to manage stress and multitask effectively while receiving calls.

• Strong communication and critical thinking abilities.

• Must be able to work independently in a remote setting.

• Consistent attendance and proactive communication during all scheduled shifts are essential.

• Reliable internet connection is required during all scheduled shifts.
