Hospice Triage RN, PT

This is a fully remote position, open to applicants in Kansas.

📋 Description

• Conduct telephone triage for hospice patients and their families

• Evaluate patient conditions and identify suitable next steps

• Work collaboratively with on-call teams to manage care and resources

• Ensure all communications and interventions are accurately documented

• Maintain a composed and professional demeanor when addressing urgent calls

• Deliver essential after-hours support and facilitate prompt interventions for hospice patients receiving care

⛳️ Requirements

• Current multistate Registered Nurse (RN) license

• Experience in hospice, palliative, or end-of-life care is highly preferred

• Proficiency with technology and electronic medical records (EMR) for call documentation

• Capability to type documentation and notes in a fast-paced setting

• Ability to manage stress and multitask while handling calls

• Excellent communication and critical thinking abilities

• Capacity to work independently in a remote setting

• Dependable attendance, proactive communication, and reliable internet access during scheduled shifts
