Hospice Triage RN, PT

This is a fully remote position, open to applicants in Florida.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify suitable next steps.

• Collaborate with on-call teams to facilitate care and resource allocation.

• Precisely document communications and interventions.

• Maintain a composed and professional demeanor while managing urgent calls.

• Provide after-hours assistance and ensure timely interventions for hospice patients receiving services.

⛳️ Requirements

• Valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for documentation of calls.

• Ability to type documentation and notes in a fast-paced setting.

• Capability to manage stress and multitask while handling a minimum of 5 calls per hour on weekdays and up to 8 calls per hour on weekends.

• Strong communication and critical thinking abilities.

• Capacity to work independently in a remote work environment.

• Reliable attendance and effective communication during scheduled shifts.

• Consistent internet connectivity during all designated shifts.

• $2 shift differential for overnight shifts.
