Hospice Triage RN, PT

This is a fully remote position, open to applicants in Florida.

📋 Description

• Conduct telephone triage for hospice patients and their families.

• Evaluate patient conditions and identify suitable next steps.

• Work in collaboration with on-call teams to facilitate care and resources.

• Document communications and interventions with accuracy.

• Maintain a composed and professional demeanor while managing urgent calls.

• Offer essential after-hours support for the needs of hospice patients and their families.

• Coordinate prompt interventions and care for patients under hospice services.

⛳️ Requirements

• A valid multistate Registered Nurse (RN) license.

• Experience in hospice, palliative, or end-of-life care is highly preferred.

• Proficiency with technology and electronic medical records (EMR) for call documentation.

• Capability to type documentation and notes in a fast-paced setting.

• Ability to manage stress and multitask while receiving calls.

• Strong communication and critical thinking abilities.

• Capacity to work independently in a remote setting.

• Reliable attendance, proactive communication, and dependable internet connectivity during scheduled shifts.
